So everyone basically believes that the virtual world is the real world, right? And they're always in it? And what happens when they disconnect the wires?

They're always in it.
Most of them are in it from a young age, though your new person seems to have had a life beforehand, which really doesn't make sense, because there is hardly a world to live in outside of the game...
When they disconnect the wires they're sent to the real world, Earth, where everything is going to shambles. The Hackers and the Creators are the only ones who have lives outside of the game, and maybe a few others.
